Youth Patron Registration Requirements

1. 14 years and under
2. Parent or Guardian Signature
3. Current address
4. Current phone number

Cardholder Responsibilities

Cardholders are responsible for all materials borrowed on their card. Materials irreparably damaged or lost are the responsibility of the cardholder and as such; replacement or reimbursement for the material(s) and a processing fee of $1.00 per item is required. Failure to comply can result in a class 3 misdemeanor as stated in the Colorado Revised Statutes 24-90-119.

1. Cardholders are responsible for keeping their contact information current.
2. A $.50 fee will be charged for a replacement patron card.
